School Enrollment Form
*The School Enrollment Form (SEF) satisfies the residency proofs requirements. If your child attends a school in the WHLL boundary, this form can be completed and signed by the schools administration. This satisfies the residency proof requirement for so long as a player remains at that same school. We recommend this form be used to prove residency for all players who attend a school within the WHLL boundary. If a SEF was used to prove residency for the previous season and the player registrant has not changed schools, there is no need to have a new form completed.
